Returns Merchandise Authorization Form

Please use the below form to request the return of any merchandise purchased from CCTV Camera Pros.  Items returned without first requesting an RMA # will not be accepted. Our complete warranty and return policies can be viewed by clicking the preceding link.

  • Please fill in one section below for each product that you are requesting a return. The product name and serial number can typically be found on a silver sticker on the back of the product. It will look similar to the one seen below.


    *Note: Certain products, such as cables and connectors, do not have serial number stickers on them.  For these items, please describe them as best as you can in the Item # field.  The Serial # field can be left blank for these items.  You can refer to your order confirmation email for a description of the item. Products such as cameras and recorders MUST include the serial number.
